Danielle Wert

Director, Network Development

Danielle joined Healthy Alliance because she’s passionate about building strong networks that foster healthy communities. In her role as Network Development Director, Danielle focuses on bringing multi-disciplinary partners together to discuss the value they bring to their local communities and how Healthy Alliance can strengthen that value, both within its Social Care Network and beyond. Before joining Healthy Alliance, Danielle managed a customer engagement team for HealtheConnections — a Qualified Entity of the Statewide Health Information Network of New York — managed a Rural Health Network in Oswego County, and provided care management services for individuals with developmental disabilities and at-risk of homelessness or homeless community members. Danielle has a bachelor’s degree in political science and history from Le Moyne College. In her free time, she enjoys spending time with her husband, stepson, and two dogs at various sporting events and at their camp on the golf course.
