THE MOMENTUM 3 CAPACITY BUILDING FUND

DOLLARS TO HELP LAUNCH AND EXPAND HEALTH-RELATED SOCIAL NEED (HRSN) SERVICES

Capacity building is one of the core responsibilities of the Social Care Network (SCN) lead entity, which includes activities to further enhance the network’s ability to improve access to HRSN services. Per New York State guidelines, SCN lead entities may provide eligible CBOs with funding specifically to support capacity building for the CBO to more effectively deliver HRSN services.

What is it?

The Momentum 3 Fund is for direct investments to support new, incremental, allowable expenses that include: New Positions, Purchasing Equipment, Renting Office Space, and/or Other Requests that are directly tied and fully dedicated to HRSN service provision (screening, navigation, enhanced care management, housing, nutrition, and transportation).

Who is eligible for dollars?

Organizations must be a not-for-profit (501 (c)(3) or 501 (c)(4)) working at the local level to meet community needs by providing HRSN services to Medicaid Members and be contracted with Healthy Alliance.

Per NYS, organizations not eligible for capacity building awards include:

  • Health systems or other health care providers as defined in the DOH SCN Operations Manual, clinics, behavioral health clinics, federally qualified health centers (FQHCs), Certified Community Behavioral Health Clinics (CCBHCs), managed care organizations (MCOs), local departments of health, and for-profit entities.
  • Health Homes, Care Coordination Organizations, and Care Management Organizations may receive capacity-building funds if certain criteria are met. Email [email protected] to learn more if this applies to your organization.

How do I get dollars?

We’ve streamlined and simplified the process for requesting capacity building dollars. Here’s what’s new:

  • We’ve done away with the formal application. To make things easier than ever for you, any organization interested in receiving their share of funding will initially complete a simplified request form. Our team will reach out to meet 1:1 to discuss your submission, and we’ll follow up with a simplified budget template thereafter.
  • Rolling application period. We know deadlines can complicate things, so you can submit your request form whenever is most convenient for you through July 2026.
  • Award payments are contingent on eligible organizations executing a HRSN Service Provider Agreement with Healthy Alliance.
  • Awardees will receive one lump sum payment, for a 12-month award period.

BE SPENT:

Dollars CAN be used for:

New Positions: Cost of new staff member (salary plus fringe benefits) dedicated to providing HRSN contracted services.

Purchasing New Equipment: Purchasing equipment essential for and fully dedicated to HRSN service provision including:

  • Technology required for a HRSN dedicated position which may include a one-time equipment purchase for a laptop, cell phone, hot spot, and/or other related equipment that is fully dedicated for the position and justified by the role/responsibility of the position.
  • Equipment tied to HRSN service provision not otherwise reimbursable by HRSN services, such as office desks, chairs, and/or cubicles fully dedicated to HRSN service provision, or other equipment such a refrigerator for food storage.

Renting New/Additional Office Space: Renting new office space fully dedicated to the HRSN service provision the organization will be contracted to provide.

Other requests: other requests fully dedicated to HRSN service provision, not covered by the New Positions, Purchasing New Equipment, or Renting New/Additional Office Space categories above.

Other Requests could include but are not limited to:

  • Startup funds for HRSN services: Up to $20,000 in funding to be used as startup funds in order to support timely delivery of services.
  • Training programs for staff: Hiring or training existing staff to provide specially licensed services, i.e., training or certification for an organization to become a Medically Tailored Meal provider or Asthma Educator.
  • Direct technical assistance for the provision of HRSN services not already existing within the organization, e.g. compliance program toolkits. This does not have to be for the explicit provision of HRSN services, but can include needs from support roles.
  • Unique requests for funding to implement and/or expand HRSN service provision that do not fall into the New Positions, New Equipment, or Renting New Office Space categories.

Dollars CANNOT be used for:

  • Vehicles
  • Recruitment expenses
  • Contracted positions
  • Ongoing monthly service charges for cell phones/hot spots
  • Existing expenses and positions not related to HRSN service provision
  • Capital expenditures/“bricks and mortar”
  • Office supplies and materials
  • Travel expenses (including mileage)
  • Costs currently funded by other local, state, and/or federal funding
  • Expenses currently being funded by capacity building dollars from another SCN
  • Partially dedicated equipment or space
  • Expenses for items already funded by Healthy Alliance for network partners

HERE’S AN EXAMPLE: A CBO is contracted with Healthy Alliance to provide pantry stocking for eligible Medicaid members. The CBO requests capacity building funding for a refrigerator to store fresh produce for the pantry stocking service, and also to hold fresh food and snacks for a locally funded community pantry program.
