Find a local provider

We work with providers you already know and trust, like your doctor, local food pantry, health plan, and more. They can help screen you if they are our partner! You can explore all the services in our network and find local providers using the searchable directory below.

To find a screening partner:

  1. Enter your location
  2. Click on ‘See All Nearby’. A list and map of local providers will come up!
  3. On the left side, click the drop down for ‘Medicaid Screening and Navigation’.
  4. Check the box that says ‘Medicaid Screening’.
  5. Nearby screening providers will appear! Reach out to them directly for a screening.
