WHO WE ARE
People have complex health-related social needs – whether it’s limited access to healthy foods, a safe place to sleep, or warm clothing – requiring streamlined coordination of care between a variety of providers. At Healthy Alliance, we lead the way in transforming how health care is delivered, shifting focus from singular medical interventions to a more proactive approach that addresses the whole needs of a person on a much larger scale.
By addressing health from all angles, we are cultivating the type of change that improves health and empowers the underserved.
WHAT WE DO
Our team, technology, and processes support successful connections between community members and our network of hundreds of local human services, behavioral, and clinical providers who offer a wide range of resources that are essential to one’s health and well-being.
The community member is always at the center of our work; our goal is to address community members’ health-related social needs – ranging from navigating and securing benefits, furnishing a home, dependable transportation, and so much more – before they turn into costly medical problems.
HOW IT WORKS
We’re an extension of our partners’ team, supporting both the evolving needs of their organization and the community members they serve. When a community member has a health-related social need, our local support team works to find and connect them, via our referral platform, to the appropriate service provider(s) that are ready and willing to help – both in and out of our network.
TOGETHER, WE ARE HEALTHY ALLIANCE
Our network brings together organizations, big and small, to coordinate and collaborate so that all communities have reliable access to the resources they need. Our mission of improving health is only made possible because of our partners, who show up every day for those who live, learn, work, and play within their communities.
We can help more people when we work together. Come see how joining our network can help both your organization and the community members you serve.

Attention New York State Medicaid Members! Are you in need of support with housing and utilities, transportation, nutrition, care management, and/or navigation to existing services?
If you answered “yes”, we have good news! As of January 1, 2025, there is a statewide Program focused on these five areas to support your health.
If you live in the Capital, Central, or North Country region* and are ready to get connected to services at no cost to you, Healthy Alliance can help. The first step is to get screened so we can better understand your needs and if you are eligible for services under this Program. Do the free screening and learn more at www.healthyalliance.org/member

Listen closely ... to find out who the 2024 Q4 recipient of our Quarterly Employee Recognition Award is! 👂
We played a game of telephone at yesterday's all-staff meeting using phrases that described this quarter's awardee until it was easy to nail down who it was. You heard it right, folks. It's someone who's always sure they can help out fellow team members — Chris Runk! 🎉
Along with celebrating this Operations team OG and superstar, the team gathered for a holiday sweater party and potluck, followed by a DEAI presentation to cap off 2024.
