WHO WE ARE
People have complex health-related social needs β whether it’s limited access to healthy foods, a safe place to sleep, or warm clothing β requiring streamlined coordination of care between a variety of providers. At Healthy Alliance, we lead the way in transforming how health care is delivered, shifting focus from singular medical interventions to a more proactive approach that addresses the whole needs of a person on a much larger scale.
By addressing health from all angles, we are cultivating the type of change that improves health and empowers the underserved.
WHAT WE DO
Our team, technology, and processes support successful connections between community members and our network of hundreds of local human services, behavioral, and clinical providers who offer a wide range of resources that are essential to oneβs health and well-being.
The community member is always at the center of our work; our goal is to address community membersβ health-related social needs β ranging from navigating and securing benefits, furnishing a home, dependable transportation, and so much more β before they turn into costly medical problems.

HOW IT WORKS
Weβre an extension of our partnersβ team, supporting both the evolving needs of their organization and the community members they serve. When a community member has a health-related social need, our local support team works to find and connect them, via our referral platform, to the appropriate service provider(s) that are ready and willing to help β both in and out of our network.
TOGETHER, WE ARE HEALTHY ALLIANCE
Our network brings together organizations, big and small, to coordinate and collaborate so that all communities have reliable access to the resources they need. Our mission of improving health is only made possible because of our partners, who show up every day for those who live, learn, work, and play within their communities.

With ~1,300 partner locations spanning 25 counties in New York State, we believe everyone should have the same opportunity to be healthy.
