WHO WE ARE
At Healthy Alliance, our purpose is to improve health and empower the underserved. Every community has its own needs, affecting the health of those who live, learn, work, and play within them. Our network brings together organizations, big and small, to coordinate and collaborate so that all communities have reliable access to the resources they need.
Why? Because every New Yorker deserves the same opportunity to be healthy.
WHAT WE DO
Our goal is to ensure every New Yorker has access to the social services they need – ranging from stable housing, healthy food, assistance with benefits navigation, clothing and household goods, transportation – and so much more.
We believe that by addressing these social needs before they turn into medical problems, it will improve the overall health of our communities, while lowering medical costs for all.

HOW IT WORKS
The work we do is largely possible thanks to our statewide referral platform, used to connect thousands of community members in need to our growing network of 580+ organizations (ranging from regional hospitals to local schools and food pantries) offering a wide range of programs and services that are essential to one’s health and wellbeing.
Our network has grown organically over the years – both in scope and geography – and will continue to do so. Just like any public utility – gas, electricity – our work won’t stop until we’ve created a collaborative network available to every New Yorker in need.
HEALTHY ALLIANCE’S IPA
As one of the first Independent Practice Associations (IPAs) in the nation focused explicitly on social determinants of health, we’ve forged a new path toward successfully building and optimizing an integrated network while contracting for social care services with managed care organizations. Our plan is to continue to build relationships on both sides of this work: growing the breadth and scope of the organizations who join the IPA, while growing the number of health plans with whom we partner.
