WHY SUCCESS STORIES MATTER
Healthy Alliance welcomes success stories from partner organizations participating in our Social Care Network (SCN) and under New York’s 1115 Waiver Program. These stories help demonstrate how housing, nutrition, transportation, Enhanced Care Management, and other services improve the health and well-being of Medicaid Members across the North Country, Capital Region, and Central New York.
